For eligible applicants, you will be instructed to take an online assessment after providing all the necessary information. If you do not have an hour to spare from this point onwards, please revisit this page when you do have the spare time.
If you are eligible to apply, the following documents will be asked from you after finishing this application:

A copy of your Commercial Drivers License.
•
A recent copy of your DMV Report (less than 30 days old).
All California applicants must provide an H-6 Report. Note: If an accident appears on your DMV Report, we will require a copy of the ACCIDENT REPORT.
•
A copy of your Social Security Card.
•
A copy of both sides of your Medical Card.
