The Scully Companies uses an online application and testing process. We require certain information and documentation in order to complete the process. The requirements vary based on the type of job you are applying for. This application and testing process can take 1-2 hours, so we recommend that you have pertinent information ready and allow yourself adequate time to complete the entire application process.
 

Personal Information:	• Previous addresses for the past 3 years. This includes apartment Number, zip code and exact length of time in particular addresses. • Highest obtained level in Grade School, High School, and College. Your last school attended will be needed as well. • Must have legal right to work in the United States and must be at least 18 years old at the time of hire. (25 years minimum age requirement for commercial drivers) • All contact information must be current. • An email address is required. This will be used to contact you for the results of your application.
Background Information:	• Convictions, misdemeanors, or felonies need to be described and dated. • For jobs that require a commercial driver's license, traffic violations and accident records need to be disclosed.
Previous Scully Employment:	• Need complete details including duration of past employment, position(s) held, location and reason for leaving.
Employment History:	• List your last 5 years of employment history (for Commercial Drivers, please provide last 10 years of commercial driving). • You are required to list the complete mailing address, street number and name, city, state, zip code and phone numbers for each of your previous employers.
Veterans Information: (this is voluntary)	• Applicable to Scully Transportation Services positions only. • Provide us with details of your military service.

 
For Commercial Drivers only: If your application meets our minimum requirements and you pass our screening process, you will be required to provide the following documents:  

•	A copy of your Commercial Drivers License.
•	A recent copy of your DMV Report (less than 30 days old per DOT). All California applicants must provide an H-6 Report. Note: If an accident appears on your DMV Report, we will require a copy of the ACCIDENT REPORT.
•	A copy of both sides of your Medical Card per DOT requirements.
•	Download the Safety Performance History Records Request Form and ensure that it is completed by your current or most recent employer.

 
Any false, misleading or incomplete statement of the information requested in this application shall be sufficient grounds for disqualification of your application or discharge from employment.
